Este índice no se usa solo para esta consulta en particular, hay otras consultas que usan este mismo índice, por lo tanto, las columnas INCLUIDAS.

Conclusión
Apliqué mi índice en el sistema LIVE y actualicé mi procedimiento almacenado para usar SMALLDATETIME, a fin de hacer coincidir los tipos de datos en la base de datos para las columnas involucradas.
Después de eso, al mirar el plan de consulta, veo la imagen a continuación:
Era exactamente como quería que fuera.
Creo que el optimizador de consultas en este caso hizo un buen trabajo para obtener el mejor plan de consultas en ambos entornos, y me alegro de no haber agregado ninguna sugerencia de consulta.
